How to purchase a 5-device licence for use on a combination of Windows, Linux and macOS devices?

If you want to purchase a licence, it is the same licence for all platforms. The "up to 5 devices" licence covers five devices. Each of those five devices can be running a different operating system (Windows, macOS, or Linux), and the same licence key will work with all of them.
